STEP 1: The most important claim management technique is to require everything that may result in an injury and/or claim against the district be reported on the same day. This applies to incidents that are minor and easily dismissed as not likely to result in a claim against the district. To help accomplish this, the JIF provides a Model Incident Reporting Policy. - Employee Report (This is not a fillable form. Please print and give to the employee to complete.)
- Supervisor Report (Send a copy of the Supervisor’s Report to your Business Administrator and/or Safety Committee Coordinator.)
- Employee Authorization for Medical Attention (Optional Form)
- Employee Responsibility Letter (Word Document to Customize for your District)
- First Accident Report “FAR”
Workers’ Compensation Transmittal Form (Only Needed if Faxing)
