Claims Management – Workers’ Comp

STEP 1: The most important claim management technique is to require everything that may result in an injury and/or claim against the district be reported on the same day.  This applies to incidents that are minor and easily dismissed as not likely to result in a claim against the district. To help accomplish this, the JIF provides a Model Incident Reporting Policy. Download the Model Incident Reporting Policy and Procedure Here.  (Word Document to Customize for your District)

STEP 2: Please complete the forms in the following order.  Note: these forms should be downloaded to your computer and are not fillable.

  1. Employee Report (This is not a fillable form. Please print and give to the employee to complete.)
  2. Supervisor Report  (Send a copy of the Supervisor’s Report to your Business Administrator and/or Safety Committee Coordinator.)
  3. Employee Authorization for Medical Attention  (Optional Form)
  4. Employee Responsibility Letter (Word Document to Customize for your District)
  5. First Accident Report “FAR”

Workers’ Compensation Transmittal Form (Only Needed if Faxing)
